Below is a list of the forms available for download on this site and a description of each one.  Keep in mind that these forms are simply a courtesy provided by SchipperkeRescue.Net, Inc. (SRN) to any and all those involved in rescue.  Use of any of these forms does not constitute a direct relationship between the user, any rescuer, and/or SRN.  To use, simply click on the link for each one needed.

Adoption Application

Foster Application

Fosters must be pre-approved in order to be ready when a new rescue comes in.  Before submitting an application to foster a rescued Schip, please read the requirements and rules HERE.  You will also find the link to the form on that page.

Surrender Forms

When surrendering a dog, it is important for the Rescuer to know as much as possible about the dog, its health, habits, behavior, problems, and background as possible.  We are not judgmental, but we must be made aware of anything we are going to have to deal with.  Also, the more we know, the more easily we can find a good match as a new home.
There are two forms that must be provided to the Rescuer:

  1. The Bailment Agreement - filled out completely and signed.

  2. The Ownership Transfer Agreement - with Dog's name and your name(s) and signature(s), ONLY

Please note:  No dog will be accepted without completed, signed Bailment  and Transfer Agreements and all documentation available including but not limited to veterinary records, registration and/or licensing records, and/or other documents.

Adoption Contract

Each Rescuer's adoption contract is slightly different.  However, there are certain provisions that are common to them all.  Here are some of those provisions:

  • To the best of our knowledge, this dog has no defects that would make it unsuitable as a family pet.As Adopter of this animal, I understand and agree to the following terms of this contract and understand that non-compliance with the terms of this agreement gives ___________________ (rescuer/org) the right to reclaim this dog without refund of the adoption fee.

  •  

  • The dog will live in my home and not outdoors.  He/she will be cared for and loved and treated with kindness and respect

  • I will take the dog for annual checkups at a licensed veterinarian, at which time all appropriate shots and tests will be given and/or performed.

  • I will license the dog annually in our town, as required by law and abide by all state and local animal control and leash laws.

  • I will not allow the dog to roam unsupervised.  He/she will be leashed or in a fenced yard when he/she is outside.

  • Within 24 hours of taking possession of this animal, I will purchase an identification tag and secure it to his/her collar and ensure that it remains on him/her at all times.

  • I understand that _______________________(rescuer/org) makes no guarantees or warranties regarding the health or temperament of this dog or suitability for any purpose.

  • I shall hold ___________________________(rescuer/org) and it agents harmless of all liability for any damage the dog might do to property or persons.  I agree to adopt this dog and to be solely responsible for this animal and any actions resulting from its actions.

  • IF FOR ANY REASON, I/we cannot keep the dog, we will contact _______________________ (Rescuer/org), or an agent thereof who may take the dog back, or allow her/they to screen any third party who would like to adopt the dog.  Under NO circumstances will we transfer ownership of the dog without _________________________ (Rescuer/org) permission.  I understand this is a binding contract enforceable by civil law.

Dog Owner’s Assumption of Risk:  Dog owner acknowledges that ownership of the Dog poses dangers to people and animals.  Such dangers include, by way of example and not limitation, the risk that the Dog can bite, trip and knock down people, especially children, and can fight other animals.  Dog Owner also acknowledges that SchipperkeRescue.Net, Inc. has limited information about the Dog, and, therefore, cannot predict whether the Dog will be suitable for the Dog Owner’s lifestyle and household.  Accordingly, Dog Owner, on behalf of him/her, his/her spouse and minor children, assumes the risk of injuries, losses, damages, costs and expenses by any means above described, and other injuries, losses, damages, costs and expenses of every possible cause and description caused by the Dog, and agrees that SchipperkRescue.net shall not be held responsible for the same.

 Legal Provisions:  This Agreement supersedes all prior discussions, representations, warranties and agreements of the parties, and expresses the entire agreement between Dog owner and SchipperkeRescue.Net regarding the matters described above.  The parties confirm that, except for that which is specifically written in the Agreement, no promises, representations or oral understandings have been made with regard to the Dog or anything else.  Without limited the generality of the foregoing, Dog Owner acknowledges that SchipperkeRescue.Net has not represented, promised, guaranteed or warranted that the Dog can be trained, that the Dog will never bit, that the Dog will not be dangerous or vicious in the future or that the Dog will not exhibit other behavioral problems.  This Agreement may be amended only by a written instrument signed by both Dog Owner and SchipperkeRescue.Net.  Any remedy provided in this Agreement is in additional to any and all other remedies provided by law or equity.  If any provision(s) of this Agreement is invalid, void or unenforceable, they will be severed and the remaining provisions shall be given full force and effect.
